DIAGNOSTIC ASSESSMENT PROCESS EXPLAINED:

Dr Jack consults from  The Wellness Centre  at Castle Quay in St Helier, Jersey.

Consultations are conducted both face to face and via encrypted video channel.

First Stage: Prior to consultation you will be asked to complete several forms and questionnaires.  This is a necessary step, it can be tedious and frustrating. Dr Jack will assess your completed forms prior to consulting with you. You might want to request an additional computerised diagnostic test-Qb-Check at an additional fee of £ 150.

Second Stage: The first consultation typically takes about an hour and a half. When patients are accompanied by family members, it may take longer. The aim of the first consultation is to develop rapport, gather relevant history and determine diagnosis and treatment plan both short and long term.

During consultations you may be asked uncomfortable questions about past traumatic experiences, sexual function, relationship problems, substance use, weight, medical issues past and present. All these details are relevant and important.

The information about past and present use of all medication and in particular psychiatric medication is extremely important. It is always useful to know the history of your response and side effects to previous treatment.

The first consultation can be anxiety provoking, which is normal. In most cases it settles fairly quickly. It may help to jot a few details before the consultation, however  it won't be helpful to read long letters during the consultation. It is always preferred to conduct the consultation in the form of a conversation.

Third Stage; Comprehensive report.

At the end of the consultation Dr Jack will give you a summary of the conclusions drawn and a clear treatment plan. In most cases the follow up date will be agreed upon. You might be issued a prescription and / or forms for investigations, if applicable. The comprehensive report will be sent to you and to your GP or a referring professional unless you request otherwise. However it is always useful to let your GP and other professionals involved in your care know about changes in your diagnosis and treatment. You might be asked to complete additional psychometric tests, if required.

Fourth Stage; Titration of medication and monitoring.

You will be asked to complete monitoring observation forms. Dr Jack will adjust your treatment accordingly

Fifth Stage; Follow up visits and prescriptions

The follow up consultations are usually much more relaxed and are booked for half an hour. Prior to follow up appointments, you will be asked to complete a medication progress form. During these review appointments, you will be asked about your response to treatment, side-effects, new developements and general progress.
